Komen Louisiana License Plate Now Available!

Susan G Komen for the Cure's network of Louisiana Affiliates have worked together to bring to the public the Cure Breast Cancer specialty license plate!

The license plate is restricted to passenger cars, pickup trucks, vans, and recreational vehicles.

The prestige license plate is issued, upon application, to any citizen of Louisiana in the same manner as any other motor vehicle license plate.

The charge for this special license plate shall be the standard motor vehicle license tax imposed by Article VII, Section 5 of the Constitution of Louisiana, plus an annual royalty fee of twenty-five dollars for each plate, and a handling fee of three dollars and fifty cents which shall be retained by the department to offset administrative costs. The monies received from the royalty fees shall be divided equally among the Susan G. Komen Affiliate groups in Louisiana and shall be used solely for the support of programs established and administered by the Louisiana Susan G. Komen Affiliates.

Two ways to get the plate: Visit the Office of Motor Vehicle in person or visit their website to obtain the correct form and information.

WEBSITE INSTRUCTIONS:

1. Visit the Louisiana Office of Motor Vehicle Website at http://omv.dps.state.la.us/
2. Find and click the link under Vehicle Services named Special Plates or click here: http://omv.dps.state.la.us/Special%20Plates/SpecialPlates1.asp
3. Read the requirements for obtaining Special Plates then click the box [Continue]
4. Read the directions for the fee calculation then enter your current license plate number in the box them click [Submit]
5. All the available Specialty Plates will be listed in a scroll down box with their corresponding picture. Using the down arrow, scroll down to the Special Interest section and look for then click Breast Cancer Awareness.
   You should now see an image of the Susan G Komen plate to the right. Under that will be listed your fee to upgrade your plate and the period it covers. After reviewing your information click [Ordering Details]
6. Read the summary page and additional ordering details then click [ORDER FORM]
7. The next page should be the official Request for Special Plates form. Enter the rest of your personal information in the empty boxes, Name, Address, Email, and DL Number. Using your browser menu, Print the completed form and mail to the appropriate address. Be sure to enclose a copy of your current registration, proof of insurance, and check or money order for the amount listed.
